- SC LiveScan provides all services listed above
- The ORI Code on the Request for Live Scan Service will determine the level of service in addition to the standard "Rolling Fee"

What to Bring:
In order to receive Live Scan services please provide:
- 3 copies of : "Request for Live Scan Service" form
- Form provided thru employer / agency or
- http://ag.ca.gov/fingerprints/forms/BCII_8016.pdf
- Must include ORI # and DOJ Billing Code
- Valid form of ID* (Must be current / NO exceptions)
- Payment for Rolling Fee
